Can my home be modified so I can live there more safely and independently?

SCDD Description

Physical changes to the home that are needed for health, safety, accessibility, or independence, when another party is not responsible for the modification.

DDS Description

Physical modifications to a participant's home that are necessary to support health, safety, and independence, and without which the participant may be at risk of institutionalization. Services may include ramps, grab bars, widened doorways, bathroom modifications, and specialized electrical or plumbing adaptations needed to meet the participant's functional needs. Adaptations must be identified in the IPP, comply with applicable building codes, and are only provided when another party such as a landlord is not responsible for the modification. Modifications may be authorized up to 180 days prior to discharge from an institution, but are not considered complete until the participant leaves the institution and is enrolled in the waiver. General home improvements unrelated to the participant's disability-related needs are excluded.

Example
